Driver card forms
There are 3 types of personal driver cards that have different functions. These are the driver's personal cards:
- for tachograph;
- obtaining rights;
- keeping records of information about the employee-driver in order to ensure road safety.
A plastic tachograph card is issued for all drivers of vehicles that can carry more than 8 passengers, trucks, special equipment. These digital maps store all information about the drivers' work and rest hours. The right to download information from the card, which is carried out on a regular basis, is vested in both its holder and the controller serving in the passenger transportation company.
The driver card, issued during the training period and serving as the basis for issuing a license, is no longer used today. This is due to the fact that all the necessary information about the passage of training is now entered into an electronic database.
And the third type of personal driver card is just the main one used today in transport enterprises. It is about this card that will be discussed below.
The main purpose of the personal driver card
All transport operators must comply with the basic requirements for road safety. For this purpose, in accordance with Art. 20 of the Law "On Road Safety" dated 10.12.1995 No. 196-FZ, they are obliged to take care of the correct organization of the work of drivers.
In accordance with sub. 1 clause 6 of the order of the Ministry of Transport dated January 15, 2014 No. 7 to prevent the occurrence of an accident, employers - subjects of transport activities must keep records of all the necessary information about their driver workers. For this purpose, a personal driver card is issued for each employee who drives vehicles during working hours.
This card displays information:
- about qualifications;
- general driving experience in this category of transport;
- term of employment with the employer;
- driving time entrusted vehicle;
- undergoing a medical examination for intoxication;
- the presence of administrative violations in the field of traffic and the implementation of labor discipline;
- adherence to sleep and rest before an accident;
- participation in an accident;
- compliance with the conditions of the internship;
- the presence of penalties during the last year;
- interruptions in the performance of their duties as a driver.

Procedure for maintaining a document
To date, a single unified form for a personal driver card has not been approved, therefore, to enter all the necessary information about the driver, car companies use independently developed forms. Most often, the unified form T-2, approved by the decree of the State Statistics Committee of 01/05/2004 No. 1, is taken as a template.
This form is used to keep the employee's usual personal card, which contains information about work experience, qualifications and other information. To enter specific information relating only to the driver's activities, the above unified form is edited by adding the necessary tables and columns.
The personal card is filled in by a personnel officer. Notes in it are made on the basis of orders and other local acts of the company. Also, the document contains information from the documents provided by the driver about his qualifications for driving a certain type of transport, admissions and general experience. Taking into account the specifics of the driver's activities, other documents are also used as sources of information.

What is this form and when is it needed
Before disassembling the sample of filling out the T-2 form, we note that its form was approved by the decree of the State Statistics Committee of Russia dated 01/05/2004 No. 1. The document is kept for each individual employee. This is convenient because it allows the employer to have a complete employee file. The fact is that information is recorded on the card:
- date and place of birth;
- citizenship;
- education, including additional, and vocational training;
- knowledge of foreign languages;
- work experience;
- family composition;
- registration and residence address;

- holidays;
- benefits;
- other information.
All these data are accumulated by the employer in order to fulfill the obligations stipulated by the law. For example, information on the composition of the family will allow the employee's children to be included in the list of recipients of New Year's gifts or summer camps. Information about the availability of additional education or knowledge of foreign languages will make it possible to offer an employee another position or send him on a business trip abroad. Data on awards and experience are considered as the basis for sending a colleague's documents for awarding at a higher level. Hence the conclusion: an employee's personal card is a very convenient and useful source of information for employers.
Who is obliged to keep cards
Some employers believe that if the primary accounting forms given in the decree of the Goskomstat of Russia are not mandatory, then it is not worth starting them in the organization. But not everything is so simple. Indeed, the Labor Code does not directly oblige managers to maintain and fill out these forms. But in clause 12 of the government decree of 16.04.2003 No. 225 "On work books" it is said that the employer is obliged to acquaint the employee with all the changes made to the work book. The employee is obliged to confirm his consent by signing in a personal card, which duplicates all entries made in the work book.
In addition, you need to know that the T-2 form is one of the documents of military registration, according to clause 27 of the decree of the government of the Russian Federation of November 27, 2006 No. 719. Thus, maintaining this document is mandatory for all organizations and entrepreneurs.
General filling rules
A short note before discussing how to fill out the T-2 card and a sample of how to fill out each item on this document. There is a uniform form that is recommended to be used. The employer has the ability to change it, or rather, supplement it with information that, in his opinion, is lacking. At the same time, it is important to understand that approved for state level the form was developed taking into account the protection of the personal data of the citizen. And the employer, if he wants to make additions to the T-2 form, should take into account the peculiarities of the legislation in this area. The next point that you need to know is that the personal card is kept by a person authorized to maintain personnel records in the organization. The document is drawn up in paper form. If it is convenient to duplicate information from it in electronic format, you should use a computer. But so far digital signatures are not common in Russia within the framework of labor relations, and employees sign with their own hands. T-2 cards are kept on paper and all data are entered into them manually. In this regard, we note that there are no requirements for the color of the ink used.
As for the bugs and fixes, unfortunately, they cannot be avoided. If you have made an inaccuracy while filling in, the incorrect entry must be crossed out with one line (the entry must remain readable), write the correct wording next to it, then add "Believe corrected" and sign.
How to fill in points
Let's say an economist Ivan Ivanovich gets a job at Svetlyi Put LLC in February 2019. When hiring a new employee, the following documents will be required (on their basis, data is written into the form):
- identification;
- work book (if any);
- SNILS;
- military registration documents;
- documents confirming education and qualifications;
- other papers, if they are required for admission to the performance of the labor function.

When all the papers are collected, proceed with the design of the form. A fully completed document is available for download at the end of the article, but for now we will figure out how to fill out the T-2 card: a sample of filling out each item will clearly show the rules for entering information.
We start filling out the form from the header. Here you should indicate:
- full and abbreviated name of the employing organization;
- the date the card was drawn up;
- personnel number, TIN (if any) and SNILS of the new employee, his gender (abbreviated as "M" or "F");
- the nature of the work - permanent or temporary;
- type of work - main or part-time. In this cell, words must be written in full, abbreviation is unacceptable;
- the "Alphabet" field is filled in with the letter with which the employee's surname begins (in our example, it is "I").
Section I is titled “ General information". Here are indicated:
- the number of the employment contract and the date of its conclusion;
- FULL NAME. employee, date and place of his birth, citizenship;
- the degree of proficiency in a foreign language (one or more);
- information about education and profession.
Already from the second line, personnel officers have difficulties, because they do not understand which codes need to be entered and where to get them from. OKATO code - the number of the settlement (its district) in the All-Russian classifier of objects of administrative-territorial division. Changes are often made to it, so when filling out it is advisable to find out accurate information from trusted sources.
As for the abbreviation OKIN, it stands for the All-Russian classifier of information on the population. From this list of codes, which are divided into 293 categories, and take information to fill out. For the convenience of readers, we will give an example of filling out card T-2: a summary table of codes that will be needed when filling out lines 4, 5 and 6.

Information about education must be supplemented with details from the diploma: who issued it and when, in what specialty. At least one line must be filled in. If in the future the employee brings diplomas of further education, this information is added.
When filling out the tabular section, you will also need a code from the OKSO - a classifier of specialties by education. In this part, it is allowed to write words in abbreviated form, but in such a way that the recording remains understandable. For example, "im." instead of "name".
In the next line - the seventh - you must indicate the profession. To do this, you will have to use the OKPDTR - a classifier of workers' professions, employee positions and wage grades. The profession of an economist in it has a code of 27728.
In the field "Work experience" fill in all the lines (if possible). If it is not there, then only the first line is filled in - about the total experience at the time of the conclusion of the employment contract from the work book.
Let us remind you that the total length of service is the period of activity that gives the right to an old-age pension. Continuous takes into account only the period that is used when assigning benefits, and the length of service that gives the right to a seniority bonus is work in certain government organizations in certain government positions (a detailed list is presented in section II of the regulation approved by order of the Ministry of Health and Social Development of December 27, 2007 No. 808).
The data in the ninth line - "Marital status" - are indicated taking into account the codes from OKIN. The sample of filling out the T-2 card that we fill out is issued for a married man. For this reason, we have put code 2. All possible codes are shown in the table.
The next line - information about the composition of the family - is filled in from the words of the employee himself. It indicates any relatives at the request of the employer. But if the employee is married, spouses and children are usually entered. If single - parents.
Lines 11 and 12 must be filled in according to passport data. The registration address is written out from the identity card, and if it does not coincide with the actual one, then it is allowed to indicate another, but also from the words of the employee. When filling out this data, there should be no difficulties.
A lot of questions arise from personnel officers when they fill out section II of the T-2 form. Firstly, you need to understand that the document is filled out differently for recruits and those in the reserve. The conscripts have conscription certificates, and the citizens who are in reserve have military cards. On the basis of these papers, data is recorded. Let's consider each of these cases separately.
If a citizen presents a certificate, then in respect of him, section II is completed as follows:
- the first, third, fourth and seventh items do not need to be filled;
- in paragraph 2 we write that the person is subject to conscription;
- in point 5 we rewrite the information from the certificate of attribution. One of the symbols is indicated there:
- A (fit for military service);
- B (fit for military service with minor restrictions);
- B (limited fit for military service);
- G (temporarily unfit for military service);
- D (not fit for military service);
- item 6 is filled out from the assigned;
- clause 7 contains an entry if a person is booked or there is a mobilization order (stamp on extradition, withdrawal);
- the last point - 8 - is filled out if a certificate of deregistration is sent from the military registration and enlistment office.
All information specified in this section is certified by two persons: the employee himself and the personnel officer. The date of the employee's acquaintance with the document is mandatory.
If an employee is in reserve and he has a military ID, then all data is written out from him. The HR officer does the following:
- does not complete item 1;
- the rank is indicated in accordance with the data in paragraph 5 of the military card;
- in paragraph 3 indicate the composition (profile), the mention of which is in the military card, - command or soldiers. You cannot abbreviate words in this line;
- item 4 ("Full code designation of the VUS") contains both letters and numbers. This information should be copied without errors from the military ID;
- filling in items 5-8 is no different from the rules described for conscripts.
Section III (hiring and transfers) is usually straightforward, as it duplicates details from the relevant orders. The only thing to remember is that transfers to temporary work are not recorded. Also, no entry is made when the salary changes.
Sections IV-VII remain blank if the employee has not passed certification and retraining, does not have awards or honorary titles. But if the employee was sent for retraining and advanced training and brought a document on education, new information must be entered into the T-2 form - an example of filling out the type of advanced training in the T-2 card will show how to do this.
But the next section, which contains information about vacations, is filled in regularly. This section looks like this:
Note that it is necessary to enter in the table all types of vacations that are provided to a person:
- additional paid;
- annual paid;
- at own expense;
- for pregnancy and childbirth;
- for child care;
- training.
If this section runs out of space to enter all the rest periods, you need to glue the sheet and enter new information already in it.

The last section "Grounds for termination of an employment contract (dismissal)" is filled out only after the official registration of the dismissal of a person. The information from the corresponding order on termination of the employment contract is entered into it. How and how much to store T-2 forms
There are no special rules for storing personal cards of employees in the legislation. But it is agreed that they should be stored in places inaccessible to third parties. Special nomenclature folders are created for them, and cards with personal affairs and / or orders for employment are not allowed.
Another rule: keep T-2 forms of current and dismissed personnel separately. After a person is fired, the employer is obliged to keep his card for another 75 years.
Not a bad option for a taxi driver's business card. The picture, text and logo do not interfere with each other. There is a noticeable difference in the font of the phone numbers and the rest of the text. But if we assume that the business card was made by a person who is not very versed in computers and, apart from paint, does not know graphic editors, then it is quite tolerable.
Somewhat embarrassing is the phrase about cultured and sober drivers. It's just weird to imagine that a taxi driver might not be sober. It would be better if they wrote something like “Fast. Comfortable. Not expensive. "
