Driver Card Forms
There are 3 types of personal driver cards that perform different functions. These are personal driver cards:
- for tachograph;
- obtaining rights;
- maintaining a record of information about the driver for the purpose of ensuring road safety.
A plastic card for a tachograph is issued for all drivers of vehicles that can carry more than 8 passengers, trucks, and special equipment. Such digital maps store all the information about the working hours and rest of drivers. The right to unload information from the card, which is held regularly, has both its holder and the controller serving in the passenger transportation company.
The driver’s card issued during the driving training period and serving as the basis for the issuance of rights is no longer used today. This is due to the fact that all the necessary information about the training is now entered into the electronic database.
And the third version of the driver’s personal card is the main one used today at transport enterprises. This card will be discussed below.
The main purpose of a personal driver card
All subjects of transport activities must comply with the basic requirements for ensuring road safety. To this end, in accordance with Art. 20 of the Law "On Road Safety" dated 10.12.1995 No. 196-FZ, they are obliged to take care of the proper organization of work of drivers.
In accordance with sub. 1 clause 6 of the order of the Ministry of Transport dated 01.15.2014 No. 7 to prevent the occurrence of an accident, employers - transport entities must keep a record of all the necessary information about their driver employees. For this purpose, for each employee who drives vehicles during working hours, a personal driver card is issued.
This card displays information:
- about qualifications;
- general driving experience in this category of transport;
- term of employment with the employer;
- driving time entrusted vehicle;
- passing a medical examination for intoxication;
- the presence of administrative violations in the field of traffic and labor discipline;
- compliance with sleep and rest before an accident;
- participation in an accident;
- compliance with the conditions of the internship;
- the availability of penalties over the past year;
- breaks in the performance of his duties as a driver.

Document Maintenance
To date, a single unified form of a personal driver’s card has not been approved, therefore, to enter all the necessary information about the driver of a car company, they use independently developed forms. Most often, the unified form T-2, approved by the Decree of the State Statistics Committee of 05.01.2004 No. 1, is taken as a template.
According to this form, the employee’s usual personal card is kept, which contains information about seniority, qualifications and other information. In order to enter specific information concerning only the driver’s activities, the above unified form is edited by adding the necessary tables and columns.
A personal card is filled in by a personnel officer. Marks in it are made on the basis of orders and other local acts of the company. Also, the document contains information from the documents provided by the driver about his qualifications in driving a certain type of transport, tolerances and general experience. Taking into account the specifics of the driver’s activities, other documents are also used as information sources.

What is this form and when is it needed?
Before disassembling the sample for filling out the T-2 form, we note that its form was approved by Resolution No. 1 of the Goskomstat of Russia dated January 05, 2004. The document is kept for each individual employee. This is convenient because it allows the employer to have a full dossier for the employee. The fact is that information is recorded in the card:
- about date and place of birth;
- citizenship;
- education, including additional, and vocational training;
- knowledge of foreign languages;
- work experience;
- family composition;
- registration and residence address;
- relation to military service;
- certification results;
- awards;
- vacations;
- privileges;
- other information.
All these data are accumulated by the employer to fulfill the obligations prescribed by law. For example, information on the composition of the family will allow the inclusion of the children of the employee in the list of recipients of New Year's gifts or trips to summer camps. Information about the availability of additional education or knowledge of foreign languages \u200b\u200bwill make it possible to offer an employee another position or send him on a business trip abroad. The data on awards and length of service are considered as the basis for sending colleagues' documents for awarding at a higher level. Hence the conclusion: the employee’s personal card is a very convenient and useful source of information for employers.
Who is required to keep cards
Some employers believe that if the primary forms of accounting described in the resolution of the Goskomstat of Russia are not mandatory, then it is not necessary to start them in the organization. But not so simple. Indeed, the Labor Code does not directly oblige managers to maintain and fill out these forms. But in clause 12 of the government decree dated 16.04.2003 No. 225 “On work books” it is stated that the employer is obliged to acquaint the employee with all the changes made to the work book. The employee is obliged to confirm his consent by signing in a personal card, which duplicates all entries made in the workbook.
In addition, you need to know that the T-2 form is one of the military registration documents, according to clause 27 of the RF Government Decree of November 27, 2006 No. 719. Therefore, maintaining this document is mandatory for all organizations and entrepreneurs.
General filling rules
A small remark before discussing how to fill out a T-2 card, and a sample of filling in each paragraph of this document. There is a uniform form for use that is recommended. The employer has the ability to change it, or rather, supplement it with the information that, in his opinion, is not enough. At the same time, it is important to understand that the form approved at the state level was developed taking into account the protection of the personal data of a citizen. And the employer, if he wants to make additions to the T-2 form, should take into account the peculiarities of the legislation in this area. The example of filling in the personal card of a T-2 employee presented in this article is given on the basis of a unified form.
The next point that you need to know is a personal card is kept by a person authorized to maintain personnel records in the organization. The document is issued in paper form. If it is convenient to duplicate information from it in electronic format, you should use a computer. But while digital signatures in Russia are not widespread in the framework of labor relations, and employees sign themselves. T-2 cards are on paper and enter all the data in them manually. In this regard, we note that there are no requirements for the color of the ink used.
Regarding errors and corrections, unfortunately, there is no escape from them. If an error was made during filling, the incorrect record must be crossed out with one line (the record must remain readable), write the correct wording next to it, then add “Corrected Believe” and sign it.
How to fill in points
Let’s say, in February 2019, the economist Ivanov Ivan Ivanovich got a job at Svetly Put LLC. When hiring a new employee, the following documents will be required (based on them, data is written to the form):
- identification;
- employment record (if any);
- SNILS;
- military registration documents;
- documents on education and qualifications;
- other papers if they are required for admission to the performance of a labor function.
Also, the personnel officer needs a job order.
When all the papers are collected, proceed to the paperwork. A fully filled-in document is available for download at the end of the article, but for now we’ll look at how to fill out a T-2 card: a sample of filling each item will clearly show the rules for entering information.
We begin to fill out the form with the header. It should indicate:
- full and abbreviated name of the employing organization;
- date of compilation of the card;
- personnel number, TIN (if available) and SNILS of the new employee, his gender (abbreviated as “M” or “F”);
- nature of work - permanent or temporary;
- type of work - main or part-time. In this cell the words must be written in full, the reduction is unacceptable;
- the “Alphabet” field is filled with the letter with which the surname of the employee begins (in our example, “I”).
Section I is called “ General information". Here are indicated:
- number of the employment contract and the date of its conclusion;
- FULL NAME. employee, date and place of his birth, citizenship;
- degree of proficiency in a foreign language (one or more);
- information about education and profession.
Already from the second line, personnel officers have difficulties because they do not understand what codes need to be entered and where to get them from. OKATO code - the number of the settlement (its district) in the All-Russian classifier of administrative-territorial division objects. Changes are often made to it, so when filling out it is advisable to find out the exact information from verified sources.
As for the abbreviation OKIN, it stands for All-Russian Classifier of Population Information. From this list of codes, which are divided into 293 categories, and take information to fill out. For the convenience of readers, we give an example of filling out the T-2 card: a summary table of codes that will be needed when filling out lines 4, 5 and 6.

Information about education must be supplemented with details from the diploma: who and when issued it, in which specialty. Be sure to fill in at least one line. If in the future the employee brings diplomas about additional education, this information is added.
When filling out the tabular part, you will also need a code from OKSO - the classifier of specialties by education. In this part, it is allowed to write words in abbreviated form, but in such a way that the record remains clear. For example, "them." instead of “name”.
The next line - the seventh - must indicate the profession. To do this, you will have to use OKKDTR - the classifier of the professions of workers, positions of employees and tariff categories. The profession of economist in it has code 27728.
In the field “Work Experience” fill in all the lines (if possible). If it is not, then only the first line is filled in - about the total length of service at the time of the conclusion of the labor contract from the work book.
Recall that the total length of service is a period of activity that gives the right to an old-age pension. Continuous only takes into account the period that is used in the assignment of benefits, and the length of service giving the right to a bonus for the length of service is work in certain government organizations in certain public positions (a detailed list is presented in section II of the regulation approved by order of the Ministry of Health and Social Development of December 27, 2007 No. 808).
The data in the ninth row - “Marital status” - is indicated taking into account the codes from OKIN. The sample of filling in the T-2 card we are filling out is framed for a married man. For this reason, we set code 2. All possible codes are listed in the table.
The next line - information about the composition of the family - is filled out according to the employee himself. It indicates any relatives at the request of the employer. But if the employee is married, spouses and children are usually entered. If single - parents.
Lines 11 and 12 must be filled in according to the passport data. The registration address is written out from the identity card, and if it does not coincide with the actual one, then it is permitted to indicate another one, but also from the words of the employee. When filling out these data difficulties should not arise.
Personnel officers have a lot of questions when they fill out section II of form T-2. We have prepared an example of filling out information about military registration in a personal card, which will help you understand all the subtleties. Firstly, it must be understood that the document is filled out differently for draftees and those in reserve. Conscripts have certificates of conscription, and citizens who are in reserve have military tickets. Based on these papers and data is recorded. We consider individually each of these cases.
If a citizen presents a certificate, then section II shall be filled in as follows:
- the first, third, fourth and seventh paragraphs do not need to be filled out;
- in paragraph 2 we write that the person is subject to appeal;
- in paragraph 5, we rewrite the information from the registration certificate. One of the characters is indicated there:
- A (suitable for military service);
- B (suitable for military service with minor restrictions);
- In (limited to military service);
- G (temporarily unfit for military service);
- D (unfit for military service);
- clause 6 is filled out from the attribution;
- clause 7 contains a record if a person is booked or there is a mobilization order (stamp on extradition, seizure);
- the last paragraph - 8 - is filled in if a certificate of deregistration is sent from the military registration and enlistment office.
All information specified in this section is certified by two persons: the employee himself and the personnel officer. Be sure to put the date of familiarization of the employee with the document.
If an employee is in stock and has a military ID, then all data is written out of it. The personnel officer does the following:
- does not fill out paragraph 1;
- the rank is indicated in accordance with the data of paragraph 5 of the military ticket;
- in clause 3 indicate the composition (profile), the mention of which is on the military ticket - command or soldiers. You cannot abbreviate the words in this line;
- clause 4 (“Full code designation of the WMC”) contains both letters and numbers. This information should be copied without errors from the military ID;
- filling in points 5-8 is no different from the rules described for draftees.
Section III (hiring and transfers) is usually straightforward, as it duplicates details from the relevant orders. The only thing to keep in mind: transfers to temporary work are not recorded. Also do not make entries when changing the salary.
Sections IV-VII remain unfilled if the employee has not passed certification and retraining, has no awards or honors. But if the employee was sent for retraining and advanced training and brought a certificate of education, new information needs to be entered into the T-2 form - an example of filling out the type of advanced training in the T-2 card will show how to do this.
But the next section, which contains information about the holidays, is filled out regularly. This section looks like this:
Note that in the table it is necessary to enter all types of vacations that provide a person:
- additional paid;
- annual paid;
- at own expense;
- maternity;
- for child care;
- training.
If in this section the place runs out to enter all the periods of rest, it is necessary to glue the sheet and enter new information already in it.
The sections "Social benefits" and "Additional information" personnel officer should be filled out as necessary.
The last section, “Grounds for Termination of an Employment Contract (Dismissal),” is filled out only after the official registration of the dismissal of a person. The information from the corresponding order on the termination of the employment contract is entered into it. This page must be signed by the personnel officer and the employee himself.
How and how much to store T-2 forms
There are no special rules for the storage of personal cards of employees in the legislation. But it is agreed that they should be stored in places inaccessible to third parties. For them, special nomenclature folders are created, and store cards with personal affairs and (or) orders for employment are not allowed.
One more rule: T-2 forms of current and laid-off personnel are stored separately. After the dismissal of a person, the employer is required to keep his card for another 75 years.
Not a bad option for a taxi driver’s business card. The picture, text and logo do not interfere with each other. There is a noticeable difference in the font of phone numbers and the rest of the text. But if we assume that a business card was made by a person who was not very computer savvy and did not know graphic editors except for paint, then it was quite tolerable.
Somewhat embarrassed by the phrase at the expense of cultural and sober drivers. It’s just strange to imagine that a taxi driver may not be sober. It would be better to write something like “Fast. It is comfortable. Not expensive. ”

The driver’s personal card must fulfill the tasks according to the “Methodological manual for the training course for road safety specialists in road transport” (approved by the Ministry of Transport of the Russian Federation).
According to Appendix 2 to the order of the Ministry of Transport of the Russian Federation dated February 13, 2013 No. 36, a driver’s personal card for a tachograph is required from companies whose activities are related to the transportation of passengers and goods. They make it out of plastic.

- date of event;
- road accident participants;
- description of the incident;
- the consequences;
- penalties applied to the driver;
- accident registration number in the register.

- date of violation;
- description of violation;
- details of the person who revealed the fact of violation;

If a driver card is missing
The absence of a driver’s personal card does not imply specific liability. However, if during an accident there will be a fault of the driver working at a particular enterprise, the manager may have difficulties. After all, in personal card contains all the information about the employee and his violations. The fact of the lack of a card will play a negative role in the proof of the innocence of the organization in the case of an administrative offense.

According to Appendix 2 to the order of the Ministry of Transport of the Russian Federation dated February 13, 2013 No. 36, a driver’s personal card for a tachograph is required from companies whose activities are related to the transportation of passengers and goods. They make it out of plastic. A microchip is built into the card that records information about the driver and all trips. It saves information about schedules and operating modes. With it, you can monitor compliance with the regime of work and rest, high-speed mode and driving time.
